Please complete this form as fully as possible so that we may provide optimal care for your pet. If there are any existing medical notes or relevant diagnostics (x-ray images, blood work results, etc.) please email them to surgery@vssaustin.com. We suggest asking your veterinarian to copy you on the email to make sure all requested records have been emailed to our practice.
Once all of this information is received and reviewed by our medical team, we will reach out to you by phone or email with the next steps for your pet. Please keep in mind that this process takes about 1-3 business days and our medical team operates Monday through Thursday.
